Information

For medical assistance program:
1. Clients must live within the thirteen county service area around metropolitan Atlanta
2. Clients must complete the Medical Assistance Program Application (http://diabetesatlanta.org/2017/wp-content/uploads/2017/11/Map-Application-2017.pdf)
3. Clients must qualify financially with a household income at or below the poverty level as defined by the US Department of Health and Human Services.
4. Clients cannot receive third-party reimbursement or Medicaid
5. Clients must provide a copy of diabetes medication and supplies prescriptions. Prescriptions must be signed and dated within the past 12 months.
